Policies
We appreciate the opportunity to provide your healthcare
needs. These policies are intended to allow us to provide health care services
to as many patients as possible in a timely manner. Thank you for understanding
and cooperating.
Voice Mail
We have voice mail for your convenience. Leaving a message prevents being on
hold during peak hours. However, you may press (0) for the operator at any time.
Insurance Cards
To decrease insurance errors, you must present your insurance card at every visit.
If you do not, you must pay for the visit at the time of service.
Payment for services
Payment for services rendered is expected at the time of service,Including co-pays,
deductible and co-insurance. We accept cash, credit cards, or checks.
Scheduled Appointments
- Please arrive 15 minutes before your appointment time to register.
- If you are unable to keep your appointment, please call our office
the day before to cancel or reschedule. We reserve the right to charge
for missed appointments
- We do our best to see you in a timely manner when you come in for your
appointment. Therefore, if you are more than 15 minutes late, we reserve
the right to reschedule you for another time.

Prescription Refills
Prescription refills should not be an emergency. They should be handled during
scheduled office appointments or by calling the prescription refill line during
regular office hours. Refills will be filled within 3 business days (not including
holidays and week-ends). Please do not walk-in to request refills. Narcotics,
anxiety medicine and other controlled substances will only be prescribed by your
primary care physician. The On Call Physician will not call in these prescriptions.
Referrals
and Authorizations
If an authorization or pre-certification
is required by your insurance, we will need a minimum of
72 hours to contact your insurance company for authorization.
And in some cases it is necessary for you to be seen by our
physician first.
Hospitalization for our adult patients
Our practice has made the decision to offer early and evening
walk-in clinics for your convenience. With this comes the necessity to
turn our hospital care for adult patients over to a team that does this
full-time. The consultant groups we utilize are located in each of the
area hospitals and can meet the needs of our adult population in a timely
manner. Consultant groups used are based on the individual hospitals and
accept most insurance plans, they provide the specialized care that is
needed in the hospital.
Labs or other facilities and your insurance
Our
staff will attempt to determine the lab or facility that
your insurance covers, but it is ultimately your responsibility
as the patient/insured to confirm the correct lab or facility.
