What is a Medical Home?
The medical home model is a way of running a medical practice to help better coordinate care and communicate with patients. It is an extension of the traditional role of primary care.
A medical home actively seeks to coordinate care across multiple settings. This includes striving for the best communication possible with specialists and hospitals.
A medical home makes clear to patients where they should go for medical assistance at any time. This includes posting clear office hours, where to seek care after-hours, and how to communicate with the office during and after business hours.
A medical home gathers and incorporates your medical information in one place. This includes your current medications, notes from specialists, medical history, health status, test results, hospital visit information, etc.
A medical home provides evidence-based care whenever possible. This means that medical decisions are based on studies that prove that treatments are effective, rather than doctors just having to use their best judgment. We also strive to empower patients to care for themselves as much as possible by facilitating patient education and directing patients toward good medical information.
